Insurance & Medicare
At Eye Care Physicians & Surgeons, we accept most insurance coverage plans available in Oregon and we accept Medicare assignment on all Medicare claims. Please contact your insurance carrier prior to making an appointment with us, as you may be required to obtain a referral from your primary care physician. Please remember to bring your insurance card(s) with you to each appointment. Eye Care Physicians & Surgeons accepts payment by credit card, debit card, and cash or check.If you have additional questions about insurance and billings, please feel free to contact us directly or call our office at (503) 585-2022 during our regular phone hours (M-F 8am to 4pm). We will be happy to answer your questions.
Billing & Coverage
If Eye Care Physicians & Surgeons has ordered diagnostic tests, outside consultation, or any type of surgical procedure, we will contact your insurance company to verify your eligibility and perform any notification or prior authorization that may be required for those services. In addition, if you are having any type of surgical procedure (in office, outpatient, or inpatient) we will provide you with a Good Faith Estimate of your anticipated financial responsibility. This estimate will be based upon information we receive from you and your insurance company. We will do everything possible to ensure you will receive the maximum benefit from your insurance policy, but it is always advisable for patients to educate themselves about their insurance company benefits and prior authorization requirements.
